These are subject to the conditions laid out by MOH.<\/p>\n<p>&nbsp;<\/p>\n<p>Please bring your <strong>NRIC or birth certificate<\/strong>, along with your <strong>PG \/ MG \/ PA and CHAS card<\/strong>, where applicable.<\/p>\n<p>&nbsp;<\/p>\n<p>Our patients\u2019 safety is our utmost priority. Due to the COVID-19 situation, our clinic already has established work flows and different waiting zones for different groups of patients. To enhance the safety of our patients even more, do call our clinic at <strong>6458 8596<\/strong> for an appointment time to see the doctor before coming to the clinic. If you present yourself within 48 hours with a completed referral form, you will not need to pay for the consultation and swab at the receiving SASH PHPC. However, if you present to the SASH PHPC after 48 hours from your visit at the referring clinic, or if you do not bring along a completed referral form provided by the referring GP, you will need to be reviewed by the SASH PHPC clinic again. In such cases, you <strong>will<\/strong> be charged as per a regular Flu Subsidy Scheme (FSS) acute visit, if you are Singaporean \/ Permanent Resident or eligible Foreign Worker.
